 1938-41. Jews were required to fill out questionnaires in various cities/towns. They were compiled by German occupation authorities to register Jews for administrative control:  The information was used to monitor residence, issue/deny work & residence permits, assess assets, & prepare later actions, such as ghettoization & deportations. 

Krakow: USHMM notes that each Krakow questionnaire corresponded to an application for a Kennkarte (ID card).

Tarnow: For Tarnow (and other towns whose records are titled "Questionnaires of Jews from...), the records do not explicitly say that they were "formally" the ID-card applications, as exists for Krakow. Perplexity.AI notes that, "...Krakow's questionnaires are documented as ID-card applications; for Tarnow and the others that connection is likely but not formally confirmed in the sources...".

The questionnaires, digitized forms arranged alphabetically by surname's initial letter, showed German-language tabular questionnaires, left to right. You can scroll through the thumbnails under "Scans". :

2.„Name und Vorname” – Surname and given name 

3.„Geburtsdaten” – Date of birth

4. „Adresse a) frühere b) gegenwärtige” – Address: a) previous, b) current.

5. „Titel der Berechtigung” – Title of entitlement/right (often type of pension, benefit, or legal status). 

6.„Die auszahlende Anstalt” – The paying institution (the office that pays the pension/benefit).

7.„Höhe der Bezüge” – Amount of payments/benefits. 

8.Right‑side notes column – Remarks about the competent authority and file references (internal file and case numbers).
